Can I just paint over a water stain on the ceiling?

Only if the cavity above is genuinely dry, which requires measurement rather than assumption. Painting over a stain that still has moisture behind it produces a returning stain and, within a few months, an odour.

How much of the ceiling has to come out?

Only as much as is wet. We map the moisture first, because the wet area is nearly always larger than the visible stain — but the aim is to cut back to dry material and no further.
